Lakeview Scholarship application - Soroptimist International of Lakeview

Eligibility

Given to a senior woman who is graduating in June, 2024, is enrolled in or accepted to a four-year college program, and meets one of the following criteria: 1. is currently a Lakeview High School senior or 2. is a resident of Lake County School District #7 and enrolled in an on-line high school program or 3. is a resident of Lake County School District #7 and enrolled as a home-schooled student. Funds are sent directly to the college.

Instructions

1-Carefully read all information and follow the instructions in this application form; include ALL requested information. 2-Fill out the application neatly and completely; correct spelling and grammar are expected and will be considered in the assessment of your application. 3-Complete the double-spaced, typed essay, maximum 500 words, to address the following: a. chosen career path; b. goals after high school and college; and c. how you would like to positively contribute to your future communities. 4-Send two reference letters (one from an LHS teacher and one from a community member who is not family) to Barb Simpson, %Lakeview Soroptimist, P.O. Box 1128, Lakeview, OR by Saturday, May 4, 2024. 5-Sign the certification statement with your typed name. Late applications will not be accepted. When you hit “submit”, this application will be sent to the scholarship committee.

If I am selected for the Lakeview Soroptimist 2024 scholarship, I will attend the college/university and pursue the major course of study I listed on page one for the duration of the academic year. If I do not comply with that commitment, I understand I may forfeit any remaining unused portions of the scholarship award. Typed name and date will serve as a signature.
